Nephrotic Syndrome

Henoch Schonlein purpura (HSP)

Henoch Schonlein purpura is characterised by a palpable purpuric rash in association with joint, abdominal and renal symptoms.

It usually follows an upper respiratory tract infection and is commoner in autumn and winter months.

It is unusual to perform a skin biopsy in children but is typified by a leucocytoclastic angiitis.

Direct immunofluorescence shows deposits of IgA about dermal vessels:

A small proportion of children with HSP will go on to develop significant renal involvement, which commonly manifests as nephrotic syndrome.
