Management of Chronic Kidney Disease

Diffuse mesangial sclerosis

This is the second commonest cause of congenital nephrotic syndrome.

It rapidly leads to end stage renal failure, within a few months of diagnosis.

It is inherited in an autosomal recessive fashion.

Histologically, there is thickening of the glomerular basement membrane and enlargement of mesangial areas.

Affected children are normal at birth but soon develop proteinuria, which is less severe than with Finnish-type congenital nephrotic syndrome.
