The majority of children presenting with nephrotic syndrome do not require resuscitation. Those who do, are usually suffering from hypovolaemia.
Such patients should receive a bolus of 10 mls/kg of 4.5% albumin over 30 - 60 minutes. Repeat as required.
Giving 4.5% albumin is safer than giving salt-poor / 20% albumin as the latter carries the risk of over-filling the intravascular space and causing pulmonary oedema.
