High potassium concentration in the urine

A TTKG greater than 4 in the presence of hypokalaemia suggests that the potassium concentration in the cortical collecting duct has been raised.  The possible causes of this are:

  1. Mineralocorticoid activity, including aldosterone
  2. Bicarbonaturia
  3. A low chloride concentration in the cortical collecting duct fluid
